Hyperthyroidism

Signalment

  • Age: 17 years
  • Sex: female spayed
  • Breed: DMH
  • Species: cat

Disease Notes

  • Certainty: tentative
  • Comments: Crystal is a fairly easy going sweet grey tabby. She has had mild renal insufficiency for over 7 years and was started on rehmannia 8 jan of 2017. She was started on calcitriol in oct of 2017. She was eating mostly primal frozen raw and was stable.

In dec 20120 we had to stop her herbs because the other cat in the house was having chronic vomiting and GI issues and it seemed that they were worse if she got into Crystal's herbs. We were trying to figure it out.

In nov 2021 Crystal became very lethargic and anorexic for over a week. B12 was low and folate was low normal and US showed diffused GI thickening suggesting IBD. She was put on a decreasing dose of pred starting at 1/2 tab daily and decreasing to 1/2 tab every 4 days. She also started getting a good Tbsp of royal canin HP dry. I hated giving it but she sure liked it and it encouraged her to eat. She ended up gaining 1 & 1/2# over 6 months. Also in Nov 2022 her T4 was 3.9 but the free T4 was normal.

Follow up B12/folate in june 2022 was normal, but her T4 is 6.9! Her heart rate is normal, liver values are normal and Ht is stable. Her BUN/creatinine increased very slightly from 2015 to 2018, and then have decreased very slightly since then. She is happy and her appetite is good but not excessive. Thirst is normal. She likes warmth but only for a while.
Her tongue is fairly pink, and pulse is moderate. Her coast is greasy over the tailhead area, but she has always had a hard time cleaning herself and now she has a little mass/vesicle on her lower gum.

I know I need to get her off the dry food and will start that. The regular vet is urging radiation treatment. I would appreciate any suggestions you might have.
  • Final Status: no change as of day 1 of treatment

Treatment and Outcome

Problem Notes
  • IBD: First diagnosed 12/1/2021 with symptoms of anorexia, low folate and mild diffuse GI thickening. Started on pred 1/2 mg/kg BID and tapered down to 1/2 tab q 4 days by 4/2/2022, and then stopped. She did respond by eating well.
