Treatments
Day 0

Notes:  Lucas was adopted when young (1-2?) from a rescue-- where he had been surrendered for aggressive behavior. Dr. Marty Goldstein treated for rabies vaccinosis and he seemed to make a full recovery. He has not had vaccines since that time, and presents as a calm and friendly dog. Lately has presented with a group of typical geriatric issues: hind end mobility and proprioceptive problems, slightly elevated kidney labs: BUN 38.8 (9-29), Creat 1.5 (0.2-1.4), and hearing loss over past 6-8 months. Lucas has gotten periodic chiropractic care from a veterinarian in Indiana, when owner visits 2x a year. He has eaten a raw diet for years, and takes a high quality glucosamine supplement. There have been no seasonal changes in his condition, he is more stiff in the morning, he seeks warm places.

Lucas had an episode of acute RF lameness last week, was taken to local emergency clinic. Vet thought he had mainly muscle spasm and gave Gabapentin, Methacarbamol and Tramadol. Lameness resolved within 4 days of onset. Recommended to client that she taper doses off one by one in order above.

PE: body condition is moderately obese-- a 70# Anatolian shepherd. He presents with a hyperextended spine most of the time, including forward tipped pelvis, limbs are conformationally straight, sometimes stacks, sometimes camped in behind. Gait is stiff. Spine is slightly sensitive, but not remarkably so. Left tuber sacral more stiff than right. Back collapses under slight pressure, not particularly sensitive at TL joint

Treated BL 18, 23, 25, KI 3, KI 1, Bai Hui with dry needles

Diet is largely raw

Acupuncture points were stimulated

Manipulation or massage was performed


Day 17

Notes:  Owner reports improvement in mobility: " He is doing quite a bit better since he has seen you. I have stopped all the meds he was on from his trip to the emergency vet. I haven't noticed any toe dragging and he is actually picking up his back feet and not so much shuffling them anymore. He is still a little stiff but not at the level he was before. I would say his days range between 6 to 8 on a scale of 1 to 10. He isn't so cooperative about getting his nails clipped but I'm working on it."

But when making this appointment on 1/30, Owner reported some respiratory issues: occasional cough and heavy breathing, most noticed after exercise. No runny nose. Fine when quiet. We thought it best to do a cardiac evaluation to rule out congestive heart problems.

Lucas was in good spirits, but respiratory stridor increased the more time he spent in our office. Heart sounds unremarkable, no murmurs. No indication of fluid in lung fields. Marked expiratory effort, seemed to be upper airway obstruction, possibly laryngeal paralysis. We recommended chest radiograph to rule out mass or bronchial calcification.

Treated with biopuncture (traumeel, zeel, discus comp, spacupreel and neuro-rheum) at BL 13, 18, 23, 25 and Bai Hui.

Added Benefit Hips and Knees for Minor Bupleurum moving actions and pain relief.

Diet is largely raw

Day 18

Notes:  Radiology report for chest films: no mass, no calcification, no fluid in lungs. Lots of fat in mediastinal area and spondylosis noted mid-thoracic. RV thought laryngeal paralysis based upon observation during sedation.


Day 24

Notes:  We decided to check for endocrine and metabolic disease through a panel at Hemo-pet. It was difficult to pull blood-- he was very wiggly, and got progressively more dyspneic over the time he was in the office. He did not seem distressed, acted normally friendly and interested in his surroundings, but was breathing very heavy and at some points, tongue was purplish. We wondered if he could be having allergic reaction to anything in office, and gave homeopathic acute remedy Lachesis for breathing. Going outdoors did not improve obviously, but after putting him in his car, lying down, he seemed to be doing better. Owner checked in with us after her hour drive home and he was doing fine.

Day 28

Notes:  No significant abnormalities on Hemopet other than known renal elevation. Alk Phos was 222, but they deem that fairly unremarkable for a dog his age. Report uploaded.

Lucas is much better with respiratory problems, and doing pretty well. Dr. Dayon suggested a urinalysis might be helpful in more accurately assessing renal function. She will have that done by a more local vet.

Day 49

Notes:  Owner reports that Lucas is doing well: still a little stiff in the morning, but not toe dragging any more. His breathing has been fairly normal except for a couple of dyspneic episodes, each of which only lasted a minute or two. Recent urinalysis showed SG 1.018 and pH 6. His urine dribbling has improved somewhat: less frequent and less volume. Owner is wondering whether to stick with this program or make any changes.
