Diarrhea, Chronic In A Dog

Hey Steve. I have discussed this case with you before. Sophie is a 10 yr, s/f, Cocker that came to me for chronic skin and ear infections. She was the typical damp heat Cocker with yeast otitis and damp, thickened skin with recurring infections. She also has KCS and has had a CCL rupture that was repaired. Tongue was red but now dark pink, normal size and some whitish coat. Her pulse was slippery but is now pretty normal to a bit wiry. I had her stop with the dry kibble months ago and she is now feeding a low-carb, home prepared diet. A few weeks ago she started having runny diarrhea. No vomiting. She has it several times a day and at night and strains a lot. I increased her SMS to tie and her regular vet put her on metronidazole for 14 days. Her stool firmed a bit but never back to normal. When she stopped the metronidazole the stool went back to runny liquid. I changed herbs to Zhai Bai Di HW because there was a lot of night time activity with panting, restlessness, etc. It has been a few days and the owner reports no change. Any thoughts would be most appreciated.

Hi there,

Three weeks of diarrhea is considered pathognomonic for small intestinal bacterial overgrowth (SIBO), explaining the partial benefits of metronidazole.

With a wiry pulse, you need something mobilizing and dispersing, rather than Si Miao San. Huo Xiang Zheng Qi San (Agastache Combination) might be a better option. It's a broad spectrum antimicrobial, is dispersing, and is anti-Damp. If the pulse improved in response to sedating BL 40, it's the first thing I'd try.

Otherwise, the other 'knee jerk' consideration with that pulse and history might be Bu Zhong Yi Qi Tang. If that formula were to be helpful, I'd expect a moderation of the pulse by needling SP 4 (near where many vets locate LV 3, plantar to the medialmost metatarsal).

Hope that helps. There are other options, but I suspect one of those two will help.
