The first order of business for dogs that have improved on Si Miao San is to ask the owner to change their diet to real food of some type (home cooked or raw with no grains). If they don't make that change, the Damp Heat symptoms won't resolve.
I've also learned that the anal glands are part of the upper burner. So if the Damp Heat is gone now except for the anal gland infections, because the owners have fixed the diet, then Blood moving formulas can help kick out the infection. We've been using Bu Yang Huan Wu Tang with antibiotics for some, provided evidence of Heat is now absent.
For hotter cases, we've been using Modified Xue Fu Zhu Yu Tang in tandem with their Si Miao San.
Hopefully this advice gets you somewhere!
