On exam, Prudence is extremely anxious - first exam I had to have owners leave the room and she allowed an exam, albeit very tense. Pulses are narrow, toned to taut, slightly superficial. Tongue is lavendar, narrow, flabby and hangs out of mouth, dry, with subtle phlegm strands. Coat is dull, coarse, but thick.
Assumed at this time that she was Liver Blood deficient with Liver Fire or Heart Fire?? I put her on Yi Guan Jian and Long Dan Xie Gan Tang, switched her off grains to a GF dehyd raw. Did well initially but 1 month later vomiting, eating ++ grass and dirt, and diarrhea (sulphurous odour, ++gassy, tenesmus, watery liquid stool). Her mood had improved houwever, much less jumpy. Vaginal discharge the same. Pulses had slightly better tone, able to examine her more easily with owners present. Started her on probiotics. Owners thought her energy improved but now still eating ++ grass and vomits it up so they reduced herbs.
Restarted YGJ and LDXGT and 1 mth later recheck - vaginal discharge increasing, owners claim she is now more anxious, licking her anus raw. Owners also put dog on raw diet during this time which we had not discussed (I did not feel that she was ready for raw yet) as SP deficiency was definitely becoming more prominent. They claim she starving, slways hungry. Pulses now toned, finger lifting? Tongue centrally lavender with pink edges, saggy. Greenish grey mucoid vaginal exudate, copious. At this point I was more worried about the SP and diagnosed SP Qi sinking, put her on Bu Zhong Yi Qi Tang. I week later p/c from O - still vaginal discharge and new red spots around genital area, still licking the same as before, have only worked up to 1/4 dose of herbs because afraid to overdo it.
Am I completely off on my diagnosis? THese owners are a stressy couple and this dog is their child. They are being evicted by their landlord so he can sell the house and there is a lot of stress in home.
What exactly is the vaginal discharge from a TCM perspective? Damp Heat? Should this dog be put on Si Miao San or Wei Ling Tang instead for this? SHe had been doing so well for awhile now I am second guessing myself.
