Diabetes Mellitus In A Cat

I have been seeing Marley, a 10yo MN DSH for uncontrolled diabetes. We continue to increase his insulin but he continues to be polyphagic and pu/pd. I saw him 2 months ago and he was in decent shape BCS 4/9, well muscled, and lively. His pulses were of decent tone and his tongue pinky-red with wet scalloped edges. I diagnosed him with SP Qi deficiency and Damp Heat and prescribed San Ren Tang and kept his Caninsulin at 3.5IU BID. I also got him onto a good quality raw diet which he took to well thankfully! His owner has been doing a daily BG at 2:30-3pm and his BG remains in the 20s! She waited until this week to bring him back as she has $ concerns. He now has OU moderate cataract formation and has lost a little bit of weight. He continues to have a strong appetite, pu/pd, and tongue and pulse are the same. I have increased his insulin to 4.0 and then 4.5IU. I really want to help this kitty regulate with herbs? Would Si Miao San be a better choice here? He really doesn't seem all that hot so I thought SRT was a better fit. The owner is really hard to work with and just doesn't seem to get the severity of persistent hyperglycaemia.

Hi Corinne,

It may be a matter of trying formulas and seeing which one finally kicks in. SMS may help, although the progressive weight loss suggests maybe Zhi Bai Di Huang Wan might be a good place to start.

Any chance of pancreatitis here? It's odd that a raw diet hasn't dramatically helped his blood glucose, which makes me wonder if there is something damaging his islet cells on an ongoing basis. Yi Guan Jian may help with that if the pulse is more fine and weak; or SMS if the pulse is more robust. ZBDHW goes with a robust pulse, too

If additional testing does show ongoing pancreatitis and islet cell damage, then Gymnema and Cornsilk (from Natural Path) may promote their partial regeneration
