Question Details
HSA Case
by landauvet - September 29, 2009
Hi Steve,
I wanted to clarify the management of HSA in a patient of mine. 4 year old F Golden. Breedeing dog in good health. Owner discovered a mammary mass and a shoulder pea sized mass. Primary waited until she was out of heat and removed both masses and perfomed a spay. Shoulder mass was a benign adenoma, but the mammary mass was a grade 2 Hypodermal HSA. The owner has this dog dead and burried based on what she has read. However, the dog is 30 days post surgery and doing great. Chest rads were fine and owner is waiting on an ultrasound of the abdomen in the near future.

Tongue: thin, lavender, drooly
Pulse: easy to feel and strong superficially, however hard to compress and very wiry in deep position.
Active points treated. BL-17 (T), Liv-13, Liv-3
All improved the pulse.

Herbal plan
Diet high in liver (A/D3)
Bilberry extract
XFZYT
Dang Gui Bu Xie Tang

I was not sure if DGSYS or DGBXT was more appropriate. DGBXT I know is for potentially bleeding patients and the XFZYT best for Atrial HSA. This dog is clinically normal with no evidence of hemorrhage.
Thoughts on which formula combination would be best would be greatly appreciated. I have a few other cases in a similat position, so this is a good one to clarify for me.

Thanks a lot,
Kevin Landau

Hi Kevin,

If you see Damp, then Dang Gui Shao Yao San is a better choice. In this case, I'd probably give it a try given the drooling. Dang Gui is hemostatic, so you'll get some of that effect.

Your overall protocol is excellent. I'd consider adding San Leng and E Zhu to the XFZYT for a while, to better safeguard against tumor re-growth. If you obtained good margins, then the dog may end up living a normal lifespan with herbal support.

Good luck!

Steve
