Penile Discharge In A Dog

Watson is a 15 mth old neutered male German Shepherd. He has recently re-developed penile discharge after a long history of illness being off and on abx. The discharge is tan and he leaves droplets all over the house. He was neutered last June 2022, then really sick that July and on abx after developing a pharyngeal abscess that progressed to pneumonia. On abx the dripping cleared up. He has had abx on and off last 12 weeks again via a specialist. Then last 3 weeks, since off abx, dripping more again. Looks like pus, cleans himself, licks and chews at penis. Urination WNL. Will go a long time without urinating, stream seems normal. Fed BID Purina Proplan dry and Iams or Purina wet. Super finicky eater, esp while sick. Started eating much better with abx, then off again since off. Generally a nervous boy, jumpy, esp at night. Mostly good with dogs, goes to puppy daycare, reactive occas on-leash. With people overall indifferent/reserved, but never aggressive.

On exam today he is quite thin, BCS 3.5/5, shy, penis and sheath NAF, and tongue pinkish-purple, sl swollen, not overly moist; pulses wiry but toned. O/wise PE unremarkable.

It seems like a Spleen Qi deficiency with Damp Heat (or just Dampness?) with Blood deficiency. Would you think I am on the right track with DGSYS? Or is WLT better to address this?

Hi Corinne,

My thinking is that because this has happened or become aggravated two summer/falls in a row, that it could be a Shao Yang disharmony.

In western terms, treating the Shao Yang means normalizing the immune response, which could be lacking in a dog that has repeat infections. There's no immune excess here, but there is likely immune deficiency, which XCHT also treats.

If we pair it up with Bu Zhong Yi Qi Tang, then we have the Spleen Qi support you're looking for and we beef up local immunity at the skin level even further. The fact that the prepuce is involved fits with a lower burner excess that both formulas combine to relieve. Meanwhile, the upper burner and lower burner are harmonized to subside the anxiety when we use the two together.

Hopefully that helps, but your instincts are great, so go with your gut if the above doesn't ring true to you!

That does make more sense, especially since his pulses are so toned. I think I got fooled by the poor diet (which we talked a lot about and she is at least considering dehydrated raw food). I will see if I can get him on both formulas as I like the combo idea!
