T Zone Lymphoma In A Dog
by fdzenitsky - February 14, 2023    View Case Report
Hi Steve!

8YO NM Golden I just diagnosed w/ T-zone LSA. He has persistent, shifting LN enlargement since last summer, some shifting lameness, but not lame now. He has had, until I took over his case, the typical American dog life, poor diet, over-vax, etc. and a long history of Dampness accumulation and Damp Heat - over-weight, colitis, dermatitis, sebaceous cysts, lipoma, cystitis, and otitis. Also severe anxiety, an unexplained bout of persistent thrombocytopenia in 2020 (resolved), and a mild heart murmur. Currently, serum chem looks great; he has a leukocytosis w/ ^LO & MO. T is dark pink, fleshy; P is deep, toned, and deficient for his size.

He has responded beautifully to a raw diet and herbal detox started in December while I chased a vector pathogen (he is Lyme+ since last summer, tx w/ doxy x 2) and has never looked or felt better. He has no GI issues, has lost weight, graying coat is golden again, and is full of energy, much less anxiety. So, a lot Damp/Heat and Spleen def. at his core, but I don't see a lot of that now and assess the T-zone LSA as more Blood Stasis d/t it's indolent nature, so not sure where to start w/ herb selection. He has been on prednisone, 20mg SID, for many months. When I backed him down to 10mg, the LNs plumped up. Owners report no side effects from the steroid.

What is the consensus on starting Damp Heat protocol for LSA that is indolent (SMS + Hoxsey) along w/ XCHT w/ Qing Dai, Mu Li, and Xuan Shen) v Blood Stasis protocol (mod. XFZYT?, LJZT w/ a Blood tonic?) v something else? I know that palliation w/ only pred can prolong him for 2-3 years, but I would love to do more and back of the steroids as much as possible.

Thanks as always,
Franchesca

Franchesca, it sounds like you're doing an amazing job. Congratulations!

I agree you're at a crossroads where you either pursue Blood deficiency and Blood stasis; or Shao Yang disharmony. Note that persistent lymphadenopathy is a Shao Yang finding, too - and not just an indolent lesion in need of emolliating by Blood tonics and Blood movers.

Given the history of a vector pathogen, the summer onset, and the history of shifting lameness, I would try the Shao Yang approach first. A Time to Heal is about to take delivery of Natural Path's new LSA Formula, which is a recipe that Erin Bannink and I have come up with and which she has tested over the years and found to be effective. I think mid- to late next week, it should be ready for sale.

If the above doesn't help, circle back and we can look at Blood tonics and Blood movers. Note that the LSA Formula does do what pred does in Chinese medical terms. It cools and it moistens, so it should be helpful

HI Steve!

Boomer was doing well on the LSA formula, but w/in 3 days of being weaned off the prednisolone, his L carpus swelled up (this is the joint that swelled first when he was diagnosed w/ Lyme last June) to where he was not wt. bearing and some peripheral lymph nodes have puffed up as well. I had the client start the pred again yesterday and the carpal swelling has gone down. So, he seems to be dependent on steroids for now, . I am wondering about a retained pathogen in the carpal joint and elsewhere and if and how to address that w/ some Blood movers. Your thoughts?

Also, at 3 tsp./day this guy is burning through the LSA formula. It would be great to have it available in 250mg size. Meanwhile, I'd like to formulate that size if you don't mind sharing the formulation, pretty please. :)

Thanks!

Hi Franchesca,

If you have Qing Ying Tang on hand, that might be a useful addition, since it moistens and cools and moves, yet is a Shao Yang formula like the LSA Combination, which sounds like it is working at least a bit.

I often find I have to keep lymphoma animals on pred, and just find the lowest required dose. Often that is as low as 0.1 mg/kg BID

To approximate the LSA fairly easily, you can try mixing 75g of XCHT with 8g Huang Lian, 9g Qing Dai and 8g of Gua Lou Gen. That gives 100g. Multiply those figures by 2.5 to get 250g worth of LSA (or a reasonable approximation)

Hi Steve!

I have Boomer on the LSA formula, YGJ, Coriolis, and prednisolone. i had him on SMS + DGS for some digestive and steroid support, but changed to YGJ in August when he had smoldering subclinical pancreatitis and just seemed more deficient to me. He's been doing well but earlier this month developed an infected defined area of multifocal pocked lesions on his lateral thorax, biopsy came back as cutaneous lymphoma. Looks much better after a round of antibiotics and I'm now considering how to amend his therapy to include the cutaneous LSA. Would you add another oral herbal formula or just add a topical XFZYT? I'll start him on Hollywood safflower oil.

Thanks for the assist! Franchesca

Sounds like he just won't quit with his lymphoma, eh? I've found Xian Fang Huo Ming Yin topically to help cutaneous lymphoma. Maybe try it orally, too, and see if that helps you out? Otherwise, you can try your XFZYT orally and topically if it doesn't
