Feline Panleukopenia Virus In A Cat

Greetings Steve & happy holidays!

I rescued a now 14 week old kitten that developed FUO over Thanksgiving. CBC showed marked pancytopenia. I started intensive supportive care and immune support while waiting for the FUO PCR panel (it was + for FPV only). I got her temp down to normal and eating again. She is now back to being an energetic, playful, ravenous kitten with the exception of a lingering uncoordinated hind end that began 2 days into the fever and affects her movement. She has a wide abnormal gait, falls over frequently until she gets going, and climbs instead of jumps. Repeat CBC last week showed recovered WBCs and platelets and mild, regenerative anemia, almost normal range.

My understanding is that FPV affects rapidly dividing cells, such as those in the bone marrow and intestines. She never had GI signs except anorexic when febrile. The incoordination reminds me of cerebellar hypoplasia but she moved normally until she got sick, no marked intention tremors. I did note sometimes her tail quivered when straight up and that she tended to seek heat, but she's tiny and excitable, I didn't think much of either.

I would really like to try to get her mobility back. Pattern of deficiency now, no Dampness. Would you say Kidney Qi, Kidney Essence or simply Qi deficient? I am considering BWDHW or YGW may be more appropriate, maybe adding a bit of XCHT. Am I on the right track and/or do you have other suggestions?

Hi Franchesca!

Nice work rescuing that poor kitten and then getting them through this crisis! Hopefully, the neuro signs are not long-lasting. I looked on VIN and found out the cerebellar damage that causes symptoms like what you're seeing is seen in kittens infected in utero through to about 2 weeks of age. After that, the cerebellum no longer has dividing cells and should be unaffected.

From a Chinese medical perspective, I think the virus is a Shao Yin (cold) pathogen. Vulnerability to Shao Yin pathogens comes from being Kidney deficient, so maybe it's as much that which you're seeing in the hind end weakness and ataxia.

I think I would be tempted to try You Gui Wan first. It has some ability to aid spinal cord damage in case that somehow has happened, though again it shouldn't be due to FPV after two weeks of age. Do you do chiropractic? Maybe there is a coincidental issue there. If so, YGW should help stop that from recurring, too.
