Microvascular Dysplasia In A Carin Terrier
by Chase Farm - February 16, 2010
Hi Steve-
Just for saying you haven't heard from me in a while you are going to hear a lot. Thanks for responding, I am well and will catch up later. Hope you are all well- pretty cool about your gold medal in skiing. First, I ahve an 8 yr old F/Sp cairn I am seeing for herbal management of her chronic portal hepatits. She is on a home cooked diet and milk thistle and Vitamin E and denosyl, and fish oil. She takes 7.5 mg opf pred a day and is 16 lbs. SHe looked to be a liver blodd deficiency/blood stasis case and I started her on Xiao Yao San. But her enzymes continue to fluctuate between 200 and 500mg/DL (10-116) regardless of meds. I added Bao Hu Jiang Jun Tang and no improvement. Last recheck a week ago showed a lot of deep sighing, ravenous appetite, increased urination, and T-thick, swollen witha thin clear coating, P-wiry and superficial, active points BL20,40, GB24, LV13 (improved with draining at BL20 and 40). She continues to look like liver blood deficiency and stasis. I am contemplating adding Ge Xie Xue Fu Shu Yu Tang. Any other ideas? I would really like to get her off the prednisone because she has lots of Cushing' like side effects.

Hi Jean,

Thanks for watching my skiing performance. It's nice to know someone appreciates my Renaissance Man abilities.

Regarding the liver condition, if tonics don't work (such as YGJ, BZYQT, XYS), then it's time to pursue formulas that treat excess. These include XCHT and GXZYT. They can be used together if you like. I'd start with one alone, however, such as GXZYT.

The only thing that gives me pause is the points, which are consistent with Damp Heat. If it's also your perception the dog is becoming Cushingoid (another Damp Heat problem), then I would wonder if the hepatopathy is partly steroid induced, or has morphed into an inflammatory response affecting the biliary tract. In addition, we'd like the formula to be antioxidant, and support peripheral circulation (given the pulse). Rolling that all together, I'd wonder about SMS. By itself, it doesn't support that outward push to circulation, but you could add about 20g of Dang Gui or, better yet, Dang Gui Wei. That would give it the dispersing effect you're looking for, and the antioxidant anti-Cushing influence you need as well. I think that is probably what I'd try next.

Hope that helps,

Steve
