Question Details
Internal Wind/Splenic Mass
by marneemadsen - March 23, 2010
HI Steve,
14.5 yr old Am Eskimo I have been treating for a couple of years. Hx of occasional, mild seizures 1-2 times yearly. Had tremors starting 2008. Raw food since young, great attitude, appetite, exercises daily at the coast. Mild liver enzyme elevations from time to time.

Tongue is always red to red-purple and very wet to phlegm lines. Pulse is often deep, surprisingly slow for the red tongue, and with some wiriness deep, but not always. She is heat intolerant and besides her dripping tongue, has seemed Liver and Kidney Yin/Blood Deficient based on response to TMGTY, ZBDHW in the past.

She has had occasional loose stool over the years and 1-2 times a year vomiting episodes that resolve with HXZQW.

BXBZTMT + 30 gm Tian Ma and SRT + TMGTY have kept the tremors down a bit and minimal seizures - usually short, mild, on windy days and will vomit afterward. In 2008, YGJ + TMGTY stopped the tremors immediately, but she got really loose stool. Tremors are random, usually start at the head and are jerky, worse with touch or trying to eat or drink.

In fall 2009, tongue started to get more purple. Labs showed anemia 34%, AST 238, ALT 196. I added JWXYS at night.

2/10 Came in for collapse and found splenic mass. Client elected no surgery. She is still hot, so added in GXZYT, Yunnan BaiYao and bilberry.

She still gets the BXBZTMT +TM BID,XYS the morning, TMGTY at night.
I was concerned about stopping things because of the seizures/tremors.

She felt great for a month, but on the new moon she had two violent seizures at 1030 pm and 3am. They lasted 4-5 minutes with severe disorientation and restlessness all night long. She did not have her usual vomit afterward, but vomited the previous night's dinner at 5 am after drinking water. Since then has been wobbly in the the back and tremors are much worse. They are having to hand feed - she has a great appetite, but cannot get the food in without help. Still walking, but definitely not her bright self.

She has always been a challenge for me to interpret with her mixed signs. Worried the GXZYT too moving/scattering? Need more Blood/Yin but gun shy. Her stools have been perfect even on the inelegant mix of stuff now.

Today pulse in sternal recumbency is superficial, lacks tone. When standing it is just plain feeble.

Tongue is still reddish lavendar (not as red as past) and lavendar to purple underside.

PTs: BL 17 T, BL 19 S, BL 24 T, BL 20 T, TH 5 active.

I need to simplify things -
Should I move to Bu Yang Huan Wu and CHJLGMLT? How to tonify Blood/Yin?

Why the change to violent seizure?

Sorry this is so long! Thanks for your input -

Hi Marnee,

I had the same thought as you. Bu Yang Huan Wu Tang. It should help with the splenic mass, yet is used in China for stroke and seizures. It's also a way of tying the Wood parts of the case (time of onset, formerly wiry pulse, etc.) to the Metal parts of the case (collapse of Qi).

Let me know how it goes. I have a similar American Eskimo case that I may give it a go on. But the purple tongue with the feeble pulse is enough to go with BYHWT.

As for why did it happen, if it works, it will simply indicate that (in the absence of an entirely effective treatment) the dog's Qi continued to decline to a point that seizures became more violent.

Fingers crossed!

Thanks Steve,
I did send home a little CHJLGMLT last night and they said she slept through the night for the first time in a while and tremors much better.

Is ok to do both? Do I phase out the other things she is on?

Is tonifying the Qi enough to make more Blood in this case?

Thanks so much,
Marnee

I'd do one at a time, for clarity's sake.

If the CHJLGMLT improvements don't hold, try BYHWT instead. At that point, I'd probably stop the other formulas, too, if the dog has had break out seizures anyway.

And, yes, BYHWT has a pretty decent Blood tonification effect through its Qi tonifying influence. You could enhance this by adding perhaps 10g of Dang Gui to the version you're getting from Mona.

Let me know how it goes.
