Hindend Weakness In A Dog

Like most French bulldogs, Luba comes with a long history of ailments! She has hemivertebrae, luxating patellas, KCS, and has had a growth removed from her larynx when she was 5! She has had a long battle with UTIs and crystals, even on a raw diet so we're definitely dealing with some Damp Heat! According to her owner, Luba is prone to pancreatitis but she describes these episodes with lots of lip-smacking, foul smelling saliva, and slimy diarrhea that is treated with antibiotics to prevent the full-on attack. (**this sounds like Gastritis or acid regurgitation to me, ST Heat? Damp Heat?) Every Spring (Feb/March in Victoria), Luba gets black, crusty ear tips, fur falls out in chunks, and ears turn black. This has routinely been treated with Vanectyl-P every Spring. (**this seems to me to be a lack of peripheral circulation due to Qi or Blood Stasis? and Wind Invasion).

She recently came to me for help with hindend weakness (looks like early degenerative process, subtle incoordination, stumbling, difficulty or lack of strength to jump up, do stairs, etc) and furloss that is bilaterally symmetrical over her sides (her skin is black in these areas and her coat from the neck down has become coarse and dry). Blood work is all WNL.

Her tongue is lavender, wide, slightly swollen, slightly dry. Her pulses are thin, on the weak side. She is a lively happy girl, no aggressive bones in her! She even submits to every dog at the dog park and LOVES people attention. She likes soft warm beds and is very cuddly. She doesn't paddle, but will vocalize a lot in her sleep. There is no pu/pd. Fed a good quality raw diet :)

I would in the past (prior to this course) think Damp Heat and put her on Si Miao San but she wasn't overtly Hot so I diagnosed Shao Yang disharmony and started her on Xiao Chai Hu Tang for the hindend weakness, Liver Qi stagnation. I think she is quite Liver Blood deficient (KCS, coat, alopecia, vocalizing in sleep) leaning toward Qi Stagnation (Spring ear dermatitis, lip-smacking nausea, growth on larynx) however. And then with her history I wonder if there is still too much Damp Heat lingering? No cystitis or crystalluria at present though. Wondering if XCHT is enough to start her on her healing process or should I add anything else? Thinking Dang Gui Shao Yao San because it certainly fits with some of the history of Damp Heat and Liver Blood deficiency.

Hi Corinne,

I like your thinking. My suspicion is a Shao Yang disharmony, too, given the spring aggravations, and GB channel involvement (manifesting as skin lesions along the sides and ears). Shao Yang disharmony dogs are often not responsive to raw diets, even though they may still be optimal for other reasons.

The Shao Yang regulates Qi in the lower jiao, through its impact on the Dai Mai. It is also an immune regulator, so if the crystals are struvite, signaling a chronic tendency to UTIs, then harmonizing the Shao Yang could conceivably help with them, too.

Lastly, Shao Yang disharmonies are associated with pancreatitis as well as attacks of nausea, so that aspect of the history is consistent.

I agree, though, that Blood deficiency looks present. This is commonly addressed in Shao Yang disharmony dogs, by using Si Wu Tang in addition. You can even combine the two formulas in a 2:1 ratio of XCHT to SWT. Alternatively, you can try Dang Gui Shao Yao San (especially if the crystals were calcium oxalate), although its content of Dang Gui is not massive.

Hi Steve,

I started her on DGSYS for now as we didn't have SWT in. Her crystals are struvite, not calcium oxalate but she has a round little abdomen that fits along with her pulse and tongue. If this isn't enough of a Blood tonifier I will definitely add SWT!

If this isn't enough of a Blood tonifier I will definitely add SWT. Her hindend weakness and subtle incoordination are slightly worse according to owners.
