It is always difficult to think clearly with one's own dog. And this is my dog.
Penny is a 13 year old intact female 6 pound toy poodle I rescued when she was 3. About two years ago, frequency and volume of urine increased. Lab work was normal. About a year and a half ago, she developed dry/thinning coat and areas of alopecia which have become worse over time. Hair is generally thin with areas of alopecia on caudal back and tail. Again, lab work and T 4 have been normal.

Over the past 6 months, she has developed polydipsia, waking up several times for water during the night. Appetite and behavior have been normal. Also, over the past few years she has developed cataracts that are now hypermature. Her loss of vision poses minimal problems for her since she basically lives on someone's lap.

Last week she was worked up by an internist. Ultrasound was normal with the exception of the right adrenal being enlarged and irregular at the caudal pole, measuring 0.8 cm in diameter. An ACTH Stim Test showed normal pre-cortisol values and post-cortisol value of 18.6 (8.0-17.0).

Surgery is probably not an option (6 lb dog; gruesome surgery), so will remain unknown if adenoma or adenocarcinoma.

Pulse is forceful and softens with Shao Yang and Kidney points. Tongue is lavender. I was thinking of Shao Fu Zhu Yu Tang and/or Xiao Chai Hu Tang. Thoughts? Any other suggestions?

Also, Steve,on another note entirely, I am retiring April 1. Thank you so much for your wisdom and mentoring over the years. I have loved practicing this great medicine.

Wow, Deirdre! Congratulations on achieving retirement. I hope it is for all the right reasons (meaning that you have achieved financial independence). For many of us, it remains an elusive dream!

Sorry to hear about your pup's woes. I would start with Minor Bupleurum, as you suggested. Even that alone has a Kidney tonifying effect (by boosting Source Qi with its content of Ginseng). You could layer in Shao Fu Zhu Yu Tang later if results weren't what you hoped. You may actually find, though, with the thinning hair coat, that it is a Blood tonic like Si Wu Tang that you might be better off adding in. If you're on the right track, though, you should see a drop in thirst with the XCHT. At that point, you could re-assess the pulse and see if it responds to acupuncture directed Blood tonification (calling for Si Wu Tang) or relief of Blood stasis (calling for Shao Fu Zhu Yu Tang).
