Gastritis In A Dog

Daisy is an 8yo FS Daxie X Chihuahua. She came to me a couple years ago for a gurgly tummy, wanting to eat grass, on and off V/D, inappetance, anxiety, and AG issues. At the time in Nov 2022 her tongue had reddish thin edges with lavender centre, dry and pulses were deficient so I assumed Stomach and Liver Yin deficiency and put her on YGJ.

In Sept 2023 she came in for V/D again and long thin BMs. Her nose was dry, tongue lavender, dry, normal size - maybe a little swollen, and pulses decently toned. She seemed more Damp to me so I cont'd YGJ and added some WLT. All went well with this until December when she came in for borborygmi so loud O could hear across the room, then D+ that was projectile liquid (like a hose). Her tongue was now pale lavender-pink in centre, pink edges, on the dry side, normal size, maybe a little swollen, and pulses were weak, hard to find. I started her on Xiao Yao San and Probenz probiotics (do you know this one? do you like it?? probiotics that work well are so hard to find!!). Other than an AG abscess the following week she did well??? Stools tended to be formed but on the soft side and broken up however.

In June 2024, Daisy started having her frequent tummy troubles again and I had just started learning about the Gold Standard herbs and Adoptrex and it seemed to fit well. Apparently it DID NOT fit well as she had four bouts of V/D over the next mth. O did not call and was able to curb with Cerenia, but finally came in end of July distraught as she was having a hard time pooping, straining, had a reduced appetite, and had some laboured breathing. Since she was relatively bright tx'd conservatively: SQF LRS 200ml, SQ Cerenia 0.65ml, SQ B12 1.0ml, and restarted XYS.

Beg'g of August O emailed saying she had been ok, but often gassy and the borborygmi is more constant and LOUD, "even hours after getting cerenia into her and she is super uncomfortable and unsettled, seems odd since she tolerated them before". I stopped the XYS as she wasn't really pooping much, BMs when occur = a bit runny, no blood or mucous. EN ok, on walk slow and sluggish. Never sleeps away from O but last night so unsettled, restless, but no V/D. Seems more Spleen Qi and Liver Blood deficient now, more Liver Qi Stagnation occuring as well. Still needs a harmonizing formula as not particularly Damp, so started her on Bu Zhong Yi Qi Tang. As long as O could catch her before borborygmi started up at 3-4am and fed her she is fine. If O misses this and can't get her to eat, will give Cerenia. Besides the belly rumbling and some lip-licking, there is no V+ and stools overall ok. BZYQT seems to be holding her the best. After reviewing her files, I rec'd also adding Yi Guan Jian at night alongside BZYQT BID to see if we can reduce that morning nausea and improve appetite.

Recently, however, while BMs are now normal, best they’ve been in a long time and no nausea or noises, she is ITCHY!!!! Aaack!! O thinks the culprit is the Bu Zhong Yi Qi Tang. She stopped the Probenz probiotics for over a week and she was still itching, then tried stopping the herb and no itching but almost immediate tummy upset with
no related stress. Ergh! SO I've added XYS back in temporarily but I need advice!

So sorry for another long and very detailed story, but I need to make sense where I go right and wrong.

Hi Corinne,

You landed where I was going with this case: Bu Zhong Yi Qi Tang, later with Yi Guan Jian added to address the loud borborygmi and the need for food to settle the stomach.

In terms of the itch, you may just need a little Wind dispersal. In other words, the Huang Qi in Astragalus may be 'trapping the thief in the house' and ramping up inflammation against a cutaneous pathogen of some type. Since we know the dog is definitely Blood and Yin deficient, I'd suggest adding in Si Wu Xiao Feng Yin.

Hopefully that works for you. I didn't note what she was eating, but a higher protein food seems appropriate, ideally a fresh food diet. That might get to the cause of the Blood deficiency (besides the breed, anyway).

Perfect Steve, that makes so much sense! D'oh! Why didn't I think of that?!?! She is on a healthy homemade diet that I advised last year, forgot to mention.

Still really itchy unfortunately so I’ve decided to try taking her off BZYQT to see if the itch dissipates?!? My only worry is that her tummy trouble come back where she’d been so solid for several months!!!

Well, it's a good first step. If the itch subsides but tummy troubles come back, at least we will know it's a motility issue and know what other kinds of formulas might help

The Huang Qi is Heating and trapping the thief inside the house, as mentioned above, so we know we need to Clear Heat and vent a pathogen. Meanwhile we need to nourish Blood and Yin. Something that does all three is Qing Ying Tang, so I would look at that next

At this point, I am wondering about reinstituting YGJ? Just maybe not all the time. She just doesn't seem to be 100% on BZYQT alone and tends to have flare-ups. 2 weeks after her Cytopoint inj she came in with an AG abscess, then 2 weeks later she came for her next Cytopoint and hasn't needed another one since thankfully. Today though O mentioned she has become itchy again and wondered if she should give her another inj?!?!

Today she was BAR and eventually ate some beef heart treats for me. I think she was a little worried that I was going to give her more SQF and Cerenia. She is such a lovely little dog, but clearly very anxious at home and extremely sensitive. I sense she worries, esp about her mom.

Her tongue today was lavender centre, bright pink thin edges, normal size, relatively moist along centre maybe a little dry. Her pulses are fairly well toned, but not brilliant, like I could clamp them down easily. She is more overwt now, packing on the fat pads for sure. I blame this on her early spay and metabolic dysfunction/disregulation. The O has been really diligent about a homemade clean fresh diet the last 1.5 years. Her dorsum is covered in powdery flakes of white dander, but not particularly itchy. She came in 10 days ago for another Cytopoint (not my recommendation but O says she gets so irritated by the itch). SO she's definitely Liver Blood deficient, Spleen Qi deficient, Liver Qi Stagnant, but I feel like I am missing some Heat with the blood in stool. I sent her home to cont the BZYQT as well as try Halscion for the YGJ and XCHT components. I feel like there is a disconnect that could be explained by a ShaoYang disharmony, esp with her not really improving on a fresh whole food diet. I also rec'd SID feeding and to start with just feeding her at night as I feel her body needs to rest in order to restart a better metabolism?? Remind me what Cytopoint is in TCM? A Blood tonic??

O is getting more concerned every day as things went well for awhile but now seem to be worsening. Am I on the right track? How can I keep her away from Cytopoint? O also wants to start Famotidine but I really don't want this either...

Also, is there a TCM option for hormone dysregulation with early spay/neuter? I am seeing SO MANY dogs, both male and female, that are overwt even on raw or cooked and just cannot get the wt off no matter what we do!

Hi Corinne,

I think you're closer to a solution with Halscion, so hopefully it pays off a little. I was wondering about Cessorex (Xiao Chai Hu Tang plus Si Wu Tang), given the:
- response to Cytopoint (indicating immune excess rather than deficiency)
- itch despite the use of real food already (hopefully not excessively high carb)
- dorsal flaking, indicating Blood deficiency (which is common in itchy Shao Yang dogs)
- high anxiety, sensitivity and reactivity, since Yang trapped in the Yang Ming then enters the Heart
- the interior Stasis in the tongue, coupled with Heat in the exterior (indicating Yang is having issues internalizing to move Blood)
- the toned but slightly deficient pulse (common in chronic Shao Yang + Blood deficiency cases)
- the fact that the dog was good for a while (presumably the winter solstice) before worsening in spring again
- the history of colitis and GI inflammation (common in Shao Yang dogs)
If Halscion doesn't work, keep Cessorex (or Chai Hu Si Wu Tang) in mind

Also, given the weight gains and low appetite, the dog should have a full thyroid work up, but I'm guessing you did that already
